What is Hospital at Home?
Hospital at Home delivers acute-level inpatient care in a patient’s home for conditions including heart failure, pneumonia, COPD exacerbations, and cellulitis. Patients receive twice-daily in-person or virtual nursing or paramedic visits, continuous remote monitoring as needed, and around-the-clock physician oversight, all coordinated through Epic. Outcomes are comparable or superior to traditional inpatient care, at significantly lower cost.

CMS Waiver Extended Through 2030
The Acute Hospital Care at Home waiver is now locked in through 2030, giving health systems a defined runway to build, prove, and scale programs with reimbursement certainty.
